Richard L.Rovegno ANNUAL GUARDIANSHIP REPORT vice Chairman FOR PATRICIA WOOLFSON Barbara ecetary Terry L Barley June 25, 2013 Director Report from Guardian of Person and Estate Cumberland County Aging and Community Services On February 20, 2012 an emergency Guardianship hearing for Patricia Woolfson was held before Judge Al Masland. Cumberland County Aging and Community Services was appointed her Emergency Guardian of Person and Estate. At the time of the hearing, Ms. Woolfson was a patient at Holy Spirit Hospital. She was admitted falling a fall. When EMS arrived she was found on the floor in water that had overrun the sink because the faucet was on. She was found to be hypothermic and had open sores on both legs and "overpowering stench." Her open wounds needed surgical debridement or else she might have died. She refused the surgery and a psychiatrist determined her to be incapacitated. Following the hearing, the Office of Aging approved the surgery. She was later admitted to Golden Living Center-Camp Hill where she still resides. On March 12, 2012 the final guardianship hearing was held and Cumberland County Aging and Community Services was appointed her Permanent Plenary Guardian of Person and Estate by Judge Al Masland. PERSON Following the hearing Ms. Woolfson was returned to Golden Living Center-Camp Hill. Patricia was very difficult and argumentative at first but later became more compliant. Her admitting diagnoses were anemia, HTN, GERD, diverticulosis, anxiety, GI bleed and Iron deficiency. Aging staff had to clean out her apartment at Susquehanna View Apartments. In September we consented to have an endoscopy performed to see if they could.find the cause of the bleeding. We also attend the quarterly care plans where we meet with different staff of the nursing home. The Home notifies us of any changes in her condition. Although her surgeon doubted her wounds would ever entirely heal, with the wound care she received at the nursing home the wounds did heal and currently she has no major skin issues. It is the request of the Cumberland County Aging and Community Services that the agency retain Guardianship of Person for Sibyl Arms. The Golden Living Center-Camp Hill has demonstrated more than adequate quality of care for her medical problems. We believe, therefore that Ms. Woolfson should continue to reside in that nursing facility. ESTATE At time of the Guardianship Ms. Woolfson had a bank account with Integrity Bank.$17,500.00 was transferred from this account to a new Guardianship account at F&M Bank. There was $500 left in the old account to keep it open until her direct deposit can be changed to the new account. Many of these funds were used to pay her medical bills from the hospitalization. Funeral pre-arrangements were made for her with Hollinger's Funeral Home. It includes cremation with a graveside service, burial in Mt. Holly Cemetery and other associated expenses. Cost was $2500.00. Ms. Woolfson was approved for Medical Assistance starting in February 2012. Aging became her Social Security Representative Payee. Her income is $919 from Social Security and $782.90 for a total of$1701.90. Golden Living receives $1564.50 for room and board. The amount varies slightly based on number of days in the month. Ms. Woolfson receives the $45 for personal use as allowed by Medical Assistance. The Cumberland County Aging and Community Services receives $100 a month in guardianship fees as provided by the court order. Balance as of February 28, 2013 was $8248.45 Attached is a financial summary of Patricia Woolfson income and expenses.
