Magliula, is an adult individual who currently resides at 1305 Bard Drive, Shipp ens burg, Pennsylvania 17257. 2. Appellee, Commonwealth of Pennsylvania, Department of Transportation, Bureau of Drivers' Licensing is a Commonwealth agency with offices in Harrisburg, Dauphin County, Pennsylvania. 3. Appellant was cited on or about October 31, 2004, for allegedly committing a violation of the Vehicle Code. 4. It has been alleged that Appellant refused to submit to a chemical test in violation of~1547 of the Vehicle code on or about October 31, 2004. 5. By letter dated November 25, 2004, Appellee notified Appellant that his driving privilege would be suspended for a period of one (1) year effective December 30, 2004. A true and correct copy of Appellee's official Notice of Suspension is attached as "Exhibit A". WHEREFORE, your appellant respectfully requests This Honorable Court to set a hearing and deny the Commonwealth's request that his license be suspended. If yOU do not have any of these items, send a sworn nota- rized letter stating YOU are aware of the suspension of your driving privilege. You must specify in your letter why yOU are unable to return your driver's license. Remember: You may not retain your driver's license for identification purposes. PennDOT phone numbers If you do not return all current driver license pro- ducts, ~e must refer this matter to the Pennsylvania State Police for prosecution under SECT[ON 1571(a)(4) of the Pennsylvania Vehicle Code. PAYZNG THE RESTORATION FEE You must pay a restoration fee to PennDOT to be restored from a suspension/revocation of your dr/ring privilege. To pay your restoration fee, complete the following steps= 1. Return the enclosed Applicat/on for Restoration. The amount due is 1/sted un the application. 2. ~rite your dr/verls license number (listed on the first Page) on the check or money order to ensure proper credit. 5. Follow the payment and mailing instructions on the back of the appl/cation.
