Petitioner, Siyamthanda Jojo, respectfully states: -n —�i ' � rno
1. Petitioner is Siyamthanda Jojo, a minor child who resides at 431 Herman Drive, T`
Mechanicsburg, PA 17055, and is the subject of this Petition.
2. Petitioner was bom on February 2, 1996, and is currently seventeen (17) years
old.
3. Petitioner has lived with the proposed guardians, Michelle Lockwood and Steven
Lockwood, as of June 16, 2013.
4. Other individuals living with Petitioner and the proposed co-guardians are their
biological children: Clayton, 15 years old; Grace, 13 years old; James, 11 years
old; and, Samuel 9 years old. Petitioner plans to attend Grade 10 this fall at
Bible Baptist School in Shiremanstown, Pennsylvania.
5. The mother of the minor child is Nothini Jojo, who resides somewhere in Eastern
Cape, South Africa.
6. The child has been cared for Ntombizodwa Dyantyi, maternal aunt, since 2001.
7. The father of the child is unknown.
8. The child is in need of a guardian because the child's mother abandoned or
stopped taking care of him; child's mother's whereabouts are generally not
known; the child's mother failed to provide suitable housing for him; the child
lacks financial resources; the identity of the child's father is unknown; the child's
maternal aunt wishes that this child reside with the Proposed Guardians; the
local school requires legal authority to deal directly with the proposed guardian
regarding the child's education; a physician or other medical provider may require
that the proposed guardian have legal authority to obtain medical treatment for
the child; the proposed guardian wishes to have general legal authority to be able
to protect and provide for the child in any circumstances which may arise in the
future; and Proposed Co-Guardians' medical insurance requires proof of legal
guardianship.

because: Biological mother lives in distant underdeveloped area and is not in
regular contact with the child or matemal aunt. The consent of the child's
maternal aunt, with whom the child resided in South Africa, is attached as Exhibit
1.
10. The consent of the child's father is not attached because the identity of the child's
father is not known.
11. The proposed Guardians of the Person of the minor child are Michelle Lockwood,
age 43, and Steven Lockwood, age 47, of the minor child.
12. The proposed co-guardian, Michelle Lockwood, lives with Petitioner at 931
Herman Drive, Mechanicsburg, PA 17055, and is a family friend and past teacher
of the child.
13. The proposed co-guardian, Steven Lockwood, is married to Michelle Lockwood
and also resides at 931 Herman Drive, Mechanicsburg, PA 17055.
14. Criminal Record Checks of both Proposed Guardians are attached as Exhibit 2.
15. Minor child, Siyamthanda Jojo's passport is attached as Exhibit 3.
16. Minor child, Siyamthanda Jojo's F1-Visa is attached as Exhibit 4.
17. Minor Child, Siyamthanda Jojo's birth certificate is attached as Exhibit 5.
18. Petitioner believes that the proposed Guardians are qualified and suitable to be
appointed as Guardians of the Person of the minor child.
19. The consents of the proposed Co-Guardians are attached to this petition.
WHEREFORE, Petitioner, Siyamthanda Jojo, nominates Michelle Lockwood and
Steven Lockwood as Guardians of the Person of SIYAMTHANDA JOJO, and requests
this Honorable Court to make such appointment pursuant to Section 5111 of the
Probate, Estates and Fiduciaries Code, 20 Pa.C.S.A. § 5111.

Siyamthanda Jojo's maternal aunt. Siyamthanda Jojo has been under my care and
control and has resided with me continuously since 2001. My personal residence is at
1792 Flourstan Lane,Extension 26, Clayville,Olifantsfontein, South Africa 1666.
Nothini Jojo,Siyamthanda Jojo's biological.mother, has not been in regular contact with
the child during this time. Nothini Jojo lives in a rural and under-developed area distant
from my residence. I have been responsible for the child's schooling,medical treatment,
and all other legal, social,and child rearing issues for the past twelve(12) years. I affirm
and attest that pursuant to South African law, I am this child's parent. I have the legal
authority to agree and consent to the guardianship of this child.
I agree and desire that formal legal guardianship of Siyarrithanda Jcjo be legally assigned
and given to Steven and Michelle Lockwood,who reside in Cumberland County,
Pennsylvania,United States of America. Mr. and Mrs.Lockwood have known this child
for more than three(3)years and will provide him excellent education and resources and
care.

It is my understanding that Nothini Jojo intends that Steven and Michelle Lockwood
