North Middleton Authority, North Middleton Township, County of Cumberland, Commonwealth of
Pennsylvania by its solicitor, Martson Deardorff Williams Otto Gilroy & Faller, hereby files its
claim for sewer services against the real estate hereinafter described and sets forth its claim as
follows:
STATEMENT OF CLAIM
1
The name of the municipality by which this claim is filed is North Middleton Authority, a municipal
authority of the Commonwealth of Pennsylvania, located in the County of Cumberland and
organized pursuant to ordinances of North Middleton Township.

The name of the owner of the property against which this lien is filed is James Dale Roelke, 105
Cavalry Road, Carlisle, PA 17013.

A description of the property against which this Lien is filed is attached hereto and marked as
Exhibit "A" with an address of 105 Cavalry Road, Carlisle, PA 17013.

The authorities under and by virtue of which this Lien is filed is the Municipal Claims and Liens Act
of May 16, 1923, P.L. 207, as amended and supplemented, 53 P. S. 7101 et seq.

The lien is claimed for unpaid water and sewer user fees against the above - described property in the
amount of $494.03, plus any additional fees incurred thereafter.

Interest is claimed on the above - mentioned fees until paid in full.

WITNESSETH, that in consideration of One Dollar ($1.00), in hand paid, the receipt whereof is
hereby acknowledged, the said Grantors do hereby grant and convey to the said Grantee, his heirs
and assigns.
ALL THAT CERTAIN piece or parcel of land with the improvements thereon erected situate in
Township of North Middleton, Cumberland County, Pennsylvania, more particularly bounded and
described, as follows, to wit:
BEGINNING at an iron pin on the easterly side of Basin Hill Avenue, said iron pin being 102.6 feet
in a northerly direction from the northeasterly comer of the intersection of said Basin Hill Avenue
and Harriet Street and said iron pin being at the northwesterly corner of Lot No. 40 in the hereinafter
mentioned Plan of Lots; thence North 20 degrees 54 minutes West along the easterly side of said
Basin Hill Avenue, a distance of 51.3 feet to an iron pin; thence North 56 degrees East along the
southerly line of Lot No. 42 of the Plan of Lots known at Marriet Acres Extension recorded in Plan
Book No. 1, Page 101 a distance of 144.15 feet to an iron pin; thence South 34 degrees 17 minutes
East along the westerly line of Lot No. 38 in the hereinafter mentioned Plan of Lots, a distance of 50
feet to a stake; thence South 56 degrees West along said northerly line of Lot No. 40, a distance of
156 feet to an iron pin, the place of BEGINNING.
BEING Lot No. 41 in the Plan of Lots known as Marriet Acres said Plan of Lots being recorded in
the Cumberland County Recorder's Office in Plan Book 2, Page 100. Having thereon erected a
dwelling house known and numbered as 105 Cavalry Road, Carlisle, PA 17013.
BEING the same premises which George Richard Roelke, III by Deed dated January 11, 2010 and
recorded in the Office of the Recorder of Deeds in and for Cumberland County as Instrument No.
201000922, granted and conveyed unto George Richard Roelke, III, George Richard Roelke, IV, and
Christopher Andrew Roelke, Grantors herein.
This is a Corrective Deed given to correct an erroneous conveyance of the herein premises to the
previous grantees by the previous grantor and to confirm the conveyance of said premises to the
appropriate Grantee. George Richard Roelke, IV and Christopher Andrew Roelke join in this
conveyance for the sole purpose of correcting the previous erroneous conveyance and confirming the
conveyance to the appropriate Grantee James Dale Roelke.
This is also a non - taxable transfer for Pennsylvania Transfer Tax purposes between father and sons
and brothers.
And the said Grantor, George Richard Roelke, III hereby covenants and agrees that he will warrant
specially the property hereby conveyed. And the said George Richard Roelke, IV and Christopher
Andrew Roelke hereby release and quick claim the property hereby conveyed.

the Grantee to payoff a mortgage on the premises previously given by the
