The Petition of Teresa C. Sheaffer respectfully represents:
l. That she resides at 245 Creekwaod L7rive, Camp Hill, Cumberland
Cc�unty,Pennsylvania, and is the grandniece of the decedent,Robert L. Kopp.
2. That the decedent, Robert L. Kapp, died on March 4, 201 l,and at the tirne
of his death was a resident of Cumberland County, Pennsylvania,residing at Tharnwald
Home,442 Walnut Bottam Road, Garlisle, PA 17013.
3. That the decedent died testate and under Item 2 af his Last Will and
Testarnent,the Petitioner, Teresa C. Sheaffer,was appointed the persanal representative
of his estate. The ariginal of the Last Wiil and Testament of Robert L. Kapp is attached
hereto,marked as E�ibit"A", and incorporated herein by reference.
4. That the said Robert L. Kopp left surviving him the fallowing persans
entitled to share in his estate:

6. That no person is entitled to the Family Exemption.
7. That the gross value of the personal estate of the decedent does not exceed
$25,000.00.
8. That the property of the decedent and the values of the several items
thereof are as follows:
a. Checking account, No. 10007201 at Mid Penn Bank $8,919.16
b. Federal Income Tax Refund 2,060.15
c. Refund of Resident funds from Thornwald Home 50.00
d. Refund of Resident funds from Thornwald Home 1,179.66
e. Refund check from HCR Manor Care check#3775 dated 14.00
March 7, 2011
f. Death Benefit check#1358 dated March 12, 2011 from 50.00

g. $75.00 United States Saving Bond Series EE issued 122.64
in March, 1988
h. $75.00 United States Saving Bond Series EE issued 120.24

9. That the names of all unpaid claimants of whom the Petitioner has notice
or knowledge are:

10. That a schedule of assets and deductions for inheritance tax purposes will
be filed with the Register of Wills; inheritance tax assessed in the amount of$ -0-.
11. That the Petitioner and her heirs have been given written notice of the
intention to file this Petition and hereby consent to waive the requirement of the twenty
(20) days prior written notice of the intention to file such Petition. Copies of each
Waiver and Consent are attached hereto and incorporated herein by reference.
WHEREFORE, Petitioner prays that the Court direct distribution of the property
of the said Robert L. Kopp in the following manner:
l. That the Petitioner be authorized to contact Mid Penn Bank regarding
checking account No. 10007201, in the name of Robert L. Kopp and that Mid
Penn Bank be ordered to issue a check in the amount of$8,919.16 plus any
accrued interest payable to the Petitioner, Teresa C. Sheaffer, which sum of
money shall be used to pay the bills set forth in paragraph 9 of said Petition.
2. That the Petitioner be authorized to contact the Internal Revenue Service
regarding the Federal Income Tax Refund in the amount of$2,060.15 and, if
necessary, the Petitioner is autharized to request the reissuance of the Federal
Income Tax Refund of$2,060.15 payable to the Petitioner, Teresa Sheaffer,
which sum of money shall be used to pay the bills set forth in paragraph 9 of
said Petition.
3. That the Petitioner be authorized to contact Thornwald Home in Carlisle,
Pennsylvania and request Thornwald Home to reissue checks in the amount of
$50.00 and$1,179.66 payable to the Petitioner, Teresa Sheaffer, which checks
represent refunds of resident funds from Thornwald Home; and that
Thornwald Home is Ordered to reissue said checks, which sum of money shall
be used to pay the bills set forth in paragraph 9 of said Petition.
4. That the Petitioner be authorized to contact HCR Manor Care and request
HCR Manor Care to reissue check, #3775 dated March 7, 2011, in the amount
of$14.00 and that said check be made payable to the Petitioner, Teresa
Sheaffer, which sum of money shall be used to pay the bills set forth in
paragraph 9 of said Petition.
5. That the Petitioner be autharized to contact the American Legion and request
The American Legion to reissue check, #1358 dated March 12, 2011, in the
amount of$50.00 and that said check be made payable to the Petitioner,
Teresa Sheaffer, which sum of money shall be used to pay the bills set forth in
paragraph 9 of said Petition.
6. That the Petitioner, Teresa Sheaffer, be authorized to redeem a$75.00 United
States Saving Bond Series EE issued in March, 1988 with an approximate
value of$122.64; that she be authorized to execute any and all documents to
redeem the Bond and that the proceeds be made payable to the Petitioner,
Teresa Sheaffer, which sum of money shall be used to pay the bills set forth in
paragraph 9 of said Petition.
7. That the Petitioner, Teresa Sheaffer, be authorized to redeem a $75.00 United
States Saving Bond Series EE issued in April, 1988 with an approximate
value of$120.24; that she be authorized to execute any and all documents to
redeem the Bond and that the proceeds be made payable to the Petitioner,
Teresa Sheaffer, which sum of money shall be used to pay the bills set forth in
paragraph 9 of said Petition.
8. That the Petitioner be authorized to request Mid Penn Bank to issue bank
checks to the unpaid claimants set forth in paragraphs 9A 9B, 9C, 9D, 9E and
9F on the within Petition in the amounts owed;
9. That the Petitioner be authorized to close Checking account,No. 10007201 at
Mid Penn Bank after the entire proceeds have been distributed; and
10. That the Petitioner be authorized to execute any and all tax documents that
may be needed to be signed in order to comply with the laws of the
Commonwealth of Pennsylvania,�the United States of America, and any local
government.

LAST WILL AND TESTAMENT
I, ROBERT L. KOPP, single, of the Borough of Tower City, County of
Schuylkill and State of Pennsylvania, do hereby make my Last Will and Testament
and revoke all Wills by me at any time heretofore made.
ITEM I. I give, devise and bequeath my entire estate, both real and personal,
whatsoever the same may be or wheresoever the same may be situate at the time of
my decease, unto my niece, Beverly S. D'Angelo, my grandniece, Teresa C. Sheaffer,
my grandnephews, Frank B. D'Angelo and Ernest D'Angelo, and my great grandnephew,
Ernest D'Angelo, Jr. , in equal one-fifth shares. In the event that any of the said
five legatees should predecease me, then the devise and bequest of such share shall
lapse and then I give devise and bequeath my said entire estate to the survivors
of them living at the time of my decease.
ITEM II. I nominate, constitute and appoint my grandniece, Teresa C. Sheaffer,
as Executrix of this my Last ulill and Testament. I authorize and empower my said
Executrix to sell any or all of my real estate at public or private sale for such
price or prices as she may deem best and to execute, acknowledge and deliver a deed
or deeds therefor to the purchaser or purchasers thereof. I direct that my Executrix
shall not be required to enter security in any jurisdiction in which she may act .

I, ROBERT L. KOPP, single, af the Borough of Tawer City, County af
Schuylkill and State of Pennsylvania, da hereby make my Last G�ili and Testament
and revoke al1 Wills by me at any time heretofore made.
ITEM Z. I give, devise and bequeath my entire estate, bath real and personal,
whatsaever the same may be or wheresaever the same may be situate at the time of
my decease, unta my niece, Beverly S. D'Angelo, rny grandniece, Teresa C. Sheaffer,
my grandnephews, Frank B. D'Angelo and Ernest D'Angelo, and my great grandnephew,
Ernest D'Angelo, Jr. , in equal one-fifth shares. In the event that any of the said
five legatees shauld predecease me, then the devise and bequest of such share shall
lapse and then I gi.ve devise and bequeath my said enCire estate to the survivors
af them living at the tin�e of my decease.
ITEM II. T rtominate, constitute and appoint my grandniece, Teresa C. Sheaffer,
as Executrix of this my Last Glill and Testament . I autharize and empower my said
ExecuCrix ta sell any or all of my real estate at public or private sale for such
price or prices as she may deem best and to execute, ack�owledge and deliver a deed
or deeds therefor to the purchaser or purchasers thereaf. I direct that my Executrix
shall not be required to enter security in any jurisdictian in which she may act.
