1. That the Petitioner be autharized to contact Mid Penn Bank regardin�
checking account No, i 0007241, in the name of Robert L. Kopp and that Mid
Pertn Bank be ordered to issue a check in the amount of$$,919.16 plus any
accrued interest payahle to the Petitioner, Teresa C. Sheaffer, which sum of
money shall be used to pay the bills set forth in paragraph 9 of said Petition.
Z. That the Petitioner be authorized to contact the Internal Revenue Service
regarding the Federal Irtcome Tax Refiznd in the amount of$2,060.1 S and, if
necessary,the Petitianer is authorized to request the reissuance af the Federal
Income Tax Refund of$2,Ob0.15 payable to the Petitioner, Teresa Sheaffer,
which sum af money shall be used to pay the bills set farth in paragraph 9 of
said Petitian.
3. That the Petitioner be authorized to cantact Thornwald Home in Carlisle,
Pennsylvania and request Thornwald Home to reissue checks in the amount of
$54.Q0 and$1,179.66 payable to the Petitioner, Teresa Sheaffer, which checks
represent refunds of resident funds from Thornwald Home; and that
Thornwald Hame is Ordered ta reissue said checks, which sum Qf money shal�
be used ta pay the bills set farth in paragraph 9 of said Petition.
4. That the Petitioner be authorized to contact HCR Manar Care and request
HCR Manar Care ta reissue check,#3775 dated March 7, 2011, in the amaunt
of$14.Q0 and that said check be made payable ta the Petitioner, Teresa
Sheaffer, which sum of money shall be used to pay the bills set forth in
paragraph 9 af said Petition.
5. That the Petitioner be authorized to cantact the American Legion and request
The American Legion to reissue check,#1358 dated March 12, 201 l, in the
amaunt of$50.04 and that said check be made payable to the Petitioner,
Teresa Sheaffer, which sum of money shall be used to pay the bi11s set forth in
paragraph 9 of said Petition.
6. That the Petitioner, Teresa Sheaffer, be authorized to redeem a $75.00 United
States Saving Bond Series EE issued in March, 1988 with an approximate
value of$122.64; that she be authorized to execute any and all documents to
redeem the Bond and that the proceeds be made payable to the Petitioner,
Teresa Sheaffer, which sum of money shall be used to pay the bills set forth in
paragraph 9 of said Petition.
7. That the Petitioner, Teresa Sheaffer, be authorized to redeem a$75.00 United
States Saving Bond Series EE issued in April, 1988 with an approximate
value of$120.24; that she be authorized to execute any and all documents to
redeem the Bond and that the proceeds be made payable to the Petitioner,
Teresa Sheaffer, which sum of money shall be used to pay the bills set forth in
paragraph 9 of said Petition.
8. That the Petitioner be autharized to request Mid Penn Bank to issue bank
checks to the unpaid claimants set forth in paragraphs 9A 9B, 9C, 9D, 9E and
9F on the within Petition in the amounts owed;
9. That the Petitioner be authorized to close Checking account,No. 10007201 at
Mid Penn Bank after the entire proceeds have been distributed; and
10. That the Petitioner be authorized to execute any and all tax documents that
may be needed to be signed in order to comply with the laws of the
Commonwealth of Pennsylvania, the United States of America, and any local
government.
Payment and distribution are accordingly Decreed and Teresa C. Sheaffer, the
Petitioner, is hereby authorized to receive, collect and distribute the property as herein
before set forth.
