AND NOWcomes Lori Lee Jakubs, Administrator of the esta.te of Carolyn J. Arrigo, by
and through her counsel, Mark A. Mateya, Esquire, and avers the following:
1. Decedent, Carolyn J. Arrigo, died intestate on February 16, 2013.
2. Petitioner, Lori Lee Jakubs is the natural daughter of Decedent.
3. Petitioner, Lori Lee Jakubs, filed a Petition for Probate and Grant of Letters on or
about April 1, 2013.
4. Since the date of the filing of the Petition for Probate and Grant of Letters,
Petitioner filed a Petition for Settlement of Small Estate on June 11, 2013.

5. A Decree was issued by the Honorable Judge Thomas A. Placey on July 1, 2013,
granting Petitioner's Petition for Settlement of A Small Estate.
WHEREFORE, Petitioner respectfully requests that the Petition for Probate and Grant
of Letters filed to the above term and number filed on April 1, 2013,be withdrawn.
