1. Claimant is Jim Mellon General Contracting, Inc., d /b /a Mellon Certified
Restoration hereinafter referred to as "Claimant ", and filing as a
contractor.
2. Defendants /Owner is Patricia Nolan who resides at: 4714 E.Trindle Road
— Mechanicsburg, PA 17050 hereinafter referred to as Defendant /Owner.
3. The property of the "Defendants /Owner" that is subject to the mechanics'
lien claim by the "Claimant" is the property and improvements thereon
located at and commonly known as 4714 E.Trindle Road —

4. The cleaning and restoration services completed by the "Claimant" were
done pursuant to a work authorization / contract entered on January
27,2013 between "Claimant" and the "Defendants /Owner ", which is
attached here to Exhibit "A"
5. The labor and materials subject to a mechanics' lien claim pursuant to the
Mechanics' Lien Law of 1963, which were provided by "Claimant" are
those described in the estimate set forth from "Claimant" to
"Defendants /Owner ", which are attached here as Exhibit `B ".

6. Pursuant to the aforesaid contract, the "Claimant " had completed the
service on January 27, 2013 on the aforementioned property.
7. Total amount of labor & materials furnished by "Claimant ",
the "Claimant" has not been paid and is owed the sum of $1,824.67
attached as Exhibit "C ".
