Pursuant to the Order of Court dated October 31, 2012, the undersigned reports the
creation of an inherited tax-deferred retirement account with Fidelity Investments, on January 7,
2013, consisting of the minor's share of Adamadia Deforest's Tenet Healthcare Corparation
401(k) Retirement Savings Plan, also with Fidelity Investments. The initial deposit to this
account was $6,172.92.
Although this account was opened by Fidelity on January 7, 2013, the Co-Guardians did
not come into possession of the above information until August 2, 2013, whereupon they are
filing this Report, as required by the Court.
