PETITION FOR CHANGE OF NAME
AND NOW, comes Joshua Aaron Dick, a dependent child in the custody of Cumberland
County Children & Youth Services, by and through Lindsay D. Baird, Esquire, Solicitor for
CCC &YS, and pursuant to 54 Pa.C.S.A. §702, respectfully avers:
1. Joshua Aaron Dick, age 15 and dependent under the Juvenile Act, is in the custody
of Cumberland County Children & Youth Services for placement in a foster home.
2. Joshua was born on 02/17/1998 and his full name is Joshua Aaron Dick.
3. Joshua is being placed in the Subsidized Permanent Legal Custody of Carol and
Warren Jenkins, foster parents.
4. Joshua has been placed in the foster home of Carol and Warren Jenkins since
02/02/2012.
5. Joshua would like to change his last name to that of his foster parents and soon -to-
be permanent legal custodians, the Jenkins.
6. There are no judgments of record against Joshua.
WHEREFORE, Petitioner requests this Honorable Court:
a. Fix a hearing on this petition,
b. Direct that notice be given of the hearing, and
c. Enter a decree changing the youth's name from Joshua Aaron Dick to Joshua Aaron
Jenkins.

ORDER OF COURT
AND NOW, this 27th day of September, 2013, upon consideration of the attached
Petition for Change of Name, a hearing is scheduled for Tuesday, November 26, 2013, at
3:15 p.m., in Courtroom No. 5, Cumberland County Courthouse, Carlisle, Pennsylvania.
IT IS FURTHER ORDERED that notice of the filing of said Petition and the
aforesaid date, time and place of the hearing be published once in the Cumberland Law
Journal and a newspaper of general circulation in the County at least 10 days prior to the
hearing; in addition, Petitioners shall comply with all other requirements of C.C.R.P. 5,
and the Act of December 16, 1982, P.L. 1309, §6 (see 54 Pa. C.S. §701 [Historical and
Statutory Notes] [Main Volume]), and 54 Pa. C.S. §§701 et seq. Proof of these
publications must be provided at the above-scheduled hearing.
IN ADDITION, notice of the petition and time and place of the hearing shall
be provided to the child's natural parents by Petitioner at least 10 days prior to the
hearing by regular mail and by certified mail to their last known address, and proof
