agreement signed by the natural mother on or about May 11, 2013, the Court does hereby
ORDER AND DECREE the following:
1. The Court, being satisfied, finds that the best interests, needs, and welfaxe of the
minor child will be promoted by and that CHRISTOPHER S. COLLARE and COURTNEY A.
COLLARE are therefore appointed guardians of the person and estate of that minor child known
as SAMANTHA J. GRIES, born October 1, 2001, and currently age eleven (11);
2. As guardians of the minor child CHRISTOPHER S. COLLARE and
COURTNEY A. COLLARE shall have temporary legal and physical custody and all enumerated
rights and responsibilities set forth under Pennsylvania law with regard to the minor child while
she is in their care.
