Dear Register:
This letter outlines the efforts made to obtain the whereabouts and make contact with the
original subscribing witnesses to the Last Will and Testament of Vivian Eunice Ellison dated
December 19, 1991 and executed in Santa Ana, California. The original subscribing witnesses
did not have their signatures notarized. As such, we attempted to obtain their whereabouts to
have them sign an oath of subscribing witness in front of a notary.
Our first effort was made through the family of the decedent, asking if they knew the
identity or whereabouts of either of the subscribing witnesses. None of the three surviving
children knew the original witnesses. As such, out firm began to make additional efforts to
locate them. Due to the original witnesses listing an address, we first began by seazching for the
people at these locations. One witness was identified as Carmen Carrete of Orange, CA. We
obtained a phone number for her listed address. The phone book lists two other people with the
last name of Cazrete still residing at this address, but all of our efforts to make contact were met
with no response.
The address listed for the second subscribing witness was seazched, but had changed
owners multiple times since the signing of the aforementioned will in 1991. Nobody to this date
has been able to identify the name of the second signer, as the signahue is difficult to read.
Finally, we were able to contact the law firm in California, Hart, Mieras, & Morris, Inc.,
who had prepared the will for the decedent. Their legal secretary, Rachel Stephanik, seazched
their record archive, and was able to verify with Attorney Hart that he did not know who the

witnesses were, and also verified through his notes that this was a will done at a reduced cost,
because the client,the decedent in this case, asked to be able to execute the will on her own,
outside of their firm. Thus, their law firm simply drafted the will, but took no part in executing
it.
Due to our attempts at locating the original subscribing wimesses being unsuccessful, we
have executed the attached oath of non-subscribing witnesses to verify that the signature on the
Last Will and Testament of Vivian Eunice Ellison, dated December 19, 1991, is in fact the
signature of the testator.
