PETITION TO AMEND LETTERS TESTAMENTARY
TO THE HONORABLE,THE JUDGES OF THE SAID COURT:
1. Petitioner is Lauryn E. Miller, formerly Lauryn E. Stasa.
2. Petitioner is the named Executrix in the Last Will and Testament of William R.
Robinson dated November 5, 2003.
3. William R. Robinson died on May 23, 2008.
4. The said Will was offered for probate on June 26, 2008, and Letters Testamentary were
issued to Lauryn E. Miller on that same date.
5. Petitioner married Blake Stasa on October 18, 2008 and became known as Lauryn E.
Stasa.
6. A Petition was filed and a Decree of the Register of Wills was entered on August 4,
2010, amending the Letters Testamentary to reflect her name as "Lauryn E. Stasa."
7. Petitioner's marriage to Blake Stasa was terminated by Decree of divorce on August
10, 2010.
8. The administration of the Estate is not complete.
9. Petitioner is unable to transfer remaining shares of stock in Carlisle Companies
because her present identification lists her name as Lauryn E. "Miller" whereas the
Ltters Tesfarnenfary pr�sently Iist her name as Lauryn E. "Stasa."
WHEREFORE, Petitioner respectfully requests that Letters Testamentary be amended in order to
issue said Letters in her current name "Lauryn E. Miller".
