CITATIaN WITH N�JTICE
A petition has b�en filed with the Court to have you deciaxed an Incapacitated Person. If the
Court finds you to be an Incapacitated Person, your rights will be affected, including your right ta
manage money and property and to make decisions. A copy of the petition which has been filed by
HCR ManorCare Carlisle bv and throu h� their attorne�, Beniamin J. Glatfelter is attached.
�ou are hereby ordered to appear at a hearing to be held in Court Raom No. 6, Cumberland
: County Courthouse, Carlisle, Perinsylvania, on NOVEMBER 6 , 2013, at 2:30 PM. to tell the
Court why it shauld not find you to be an incapacitated Person and appoint a Guardian to act on yaur
behalf.
To be an incapacitated Person means that yau are not able to receive and
effectively evaluate inforrnation and cammunicate decisions and that you are unable to
manage your money and/or other property, ar to make necessary decisions about where
you will live, what medical care yau will get, or how your money will be spent.
At the hearing, yau have the ri�ht to appear, to be represented by an attorney, and
to request a jury trial. If you do nat have an attarney, yau have the right to request the
Court to appoint an attorney ta represent you and to have the attorney's fees paid for you
if you cannot afford to pay them yaurself. 'You al�o have the right to request that the
Court order that an independent evaluation as ta your alleged incapacity.
If the Court decides that you are an Inca.pacitated person, the Court may appoint a
Guardian for you, based on the nature af any condition ar disability and yaur capacity to

If the Caurt decides that you are an Incapacitated person,the Court may appoint a
Guardian far you, based on the nature af any condition or disability and your capacity tc�
make and com�rnunicate decisions, The�ruardian will be of your persan and/or your
maney and other property and will have either�imited or full powers to act far you.
If the caurt finds you are totally incapacitated, your legal rights will be affected
and you will not be able to make a contract or gift af your money or other property. If
the cau:rt finds thaz you are partially incapacitat�ed, your legal rights will also be limited as
directed by the Court.
If yau do not appear at the hearing{either in person or by an attorney representing you}
the court will still hold the hearing in your absence and may appoint the �uardian requested.
