Mrs. Douglas at that time was a resident of the Heritage Harbor dementia unit at Cumberland County Nursing and Rehabilitation Center. Mrs. Douglas continued to reside there until her death on July 19, 2013 at age 95. Mrs. Douglas medical diagnoses included dementia with delusions, congestive heart failure, COPD, allergies, anxiety and depression. Mrs. Douglas overall health declined over the past year. Staff provided maximum assistance with all her activities of daily living and transfers. During her personal care staff would use bath oils due to her very transparent and fragile skin. She would bruise or receive skin tears very easily even with protective coverings. Mrs. Douglas had been incontinent of bowel and bladder. Staff would cue or assist Gayle with her meals due to forgetfulness or was too busy watching what was going on that she had no interest in eating. Mrs. Douglas was on a pureed diet with pleasure soft foods such as Magic cups which tasted like ice cream. She was on a functional maintenance program for ambulation in which she was walked twice a day. She attended different activities and as time went on was more of a passive participant. One of her favorite activities was watching the men and giving them her famous wink. Her mood most of the time was very pleasant and she enjoyed laughter. The staffs at Aging and Claremont were very fond of Mrs. Douglas. On July 19, 2013 Aging received a call from Claremont informing us that Mrs. Douglas has a significant decline in her health condition. She was having difficulty breathing and was on oxygen. She also had mottling and purple feet and hands. The doctor had ordered an increase in her Lasix to reduce fluid buildup. I went over to visit her and she was in bed with an oxygen mask on resting comfortably. She was awake and after talking to her for a few minutes she winked and then laughed. I shared with her some of the visits I had with her over the years. We walked down memory lane of her being an avid Penn State fan, her life in Boiling Springs and her dog Midget. Mrs. Douglas had her eyes open and seemed to be listening. Prior to leaving we shared another laughing moment. Claremont nursing home received monthly income and gave Mrs. Douglas her monthly personal allowance as stated by medical assistance. Mrs. Douglass did not have any other financial accounts. The Department of Public Welfare's Estate recovery Division was notified of her death and any funds in her account at the nursing home.
