AND NOW, comes the claimant, TOWNSH1T OF HAMPDEN, by the President of
its Board of Commissioners, who files the following claim against Kenneth & .Toan ¥ohn, all
that certain tract of land including improvements thereon, owned by them and described
as follows:

For sewer services rendered from 07/01/01 to 01/01/02. Said services were rendered
pursuant to ordinance. Attached hereto is a statement of the kind and character of the
services so rendered and the price charged therefor, said statement being a true copy of
the owner's account with Hampden Township which sum of $216.20 was duly assessed
against the herein~bove-n~med owner and the premises on 01/0L/02 and for which sum, with
interest, penalties and costs, a lien is claimed against the above-described premises in
accordance with the Act of Mm/16, 1923, P.L. 207 ~3 P.S. Section 7101, et seq.), as
amended and supplemented, and the Act of May 2, 1945, P.L 382 (53 P.S. Section 301,
et seq), as amended and supplemented.
