NO. 13 -,5'7'70 NAME CHANGE <_' r; C,�j PETITION FOR CHANGE OF NAME AND NOW, comes Petitioner, Jesse Shane Ross,' by and through his attorney at the law firm of Saidis, Sullivan & Rogers, and files the following Petition, and in support thereof, avers the following: 1. Petitioner is Jesse Shane Ross, an adult individual currently residing at 3505 Raintree Lane, Mechanicsburg, Cumberland County, Pennsylvania 17050. 2. Jesse is the biological child of Ross Robert Reeves and Mary Dawn Hanning, formerly known as Mary Dawn Ross. 3. Ross Robert Reeves and Mary Dawn Hanning have never married and did not reside together at the time of Jesse's birth. 4. Prior to Jesse's birth, Mary Dawn Hanning was married to Daniel Ross and took his surname. 5. Mary Dawn Hanning and Daniel Ross divorced several years prior to Jesse's birth. 6. At the time of Jesse's birth he was given the surname of Ross. A copy of his birth certificate is attached hereto and incorporated, herein as Exhibit A. Law Offices of Saidis 7. Jesse has had no contact with, nor has he ever met, Daniel Ross. Sullivan 8. Jesse has resided at the following addresses during the past five (5) years: & Rogers 26 West High Street 59 Liberty Street 7/7/2002 — 5/5/2012 Carlisle, PA 17013 Portsmouth, Rhode Island 02871 3505 Raintree Lane 5/5/2012 — present Mechanicsburg, Pennsylvania 17050 9. Jesse currently lives with and resides at the home of his biological father, Ross Robert Reeves. 10. Petitioner desires and feels it would be in his best interest that his surname be changed to conform to his biological father's surname of Reeves, as he has never met Daniel Ross, and no one in his family has maintained the surname Ross. 11. Petitioner does not wish to change his name for any illegal purpose, to avoid payment of taxes or other debts, or to avoid any obligation. 12. There are no judgments or decrees of like character of record against the Petitioner. Copies of the results of judgment searches in Cumberland County, Pennsylvania, and Newport County, Rhode Island regarding Petitioner are attached as Exhibit B and incorporated herein. 13. Petitioner's fingerprint identification has been taken by the Pennsylvania State Police. The Petitioner's fingerprints are attached as Exhibit C to this Petition. WHEREFORE, Petitioner prays this Honorable Court to order that the name of Jesse Shane Ross be changed to Jesse Shane Reeves.
