PERSON
On Friday, November 17, 2006 Judge granted to the Cumberland County Office of
Aging a Temporary Emergency Involuntary Intervention court order for Elizabeth Floyd.
The order was necessary because client's caregiver died, she was being evicted from
the house and she was unable to care for herself due to dementia. She was
transported to the Holy Spirit Hospital's emergency room for medical evaluation. Upon
discharge she was admitted to dementia unit of The Todd Home, Carlisle, PA where
she is still residing.
At the time of her admission, Ms. Floyd was diagnosed with a urinary tract infection,
uncontrolled hypertension and dementia with delusions. On May 15, 2007 Judge Ebert
appointed the Cumberland County Office of Aging Permanent Plenary Guardian of
Person and Estate for Elizabeth Floyd.
Since that time she has also been diagnosed with ambulation dysfunction, atropthy, a
history of a vertebrae compression, macular degeneration, osteoperosis,
kyphoscoliosis. More recent diagnoses include rheumatoid arthritisneuromyopathy,
Korsakoff syndrome, dysarthia depression, NIDDm, afib, CAD, and GERD.she does
suffer from occasional urinary tract infections. She is seen by her physician in the
nursing home every other month. Her current medications are Lopressor 25 mg BID,
Proamatine 5mg TID, syntrhoid 50 mcg, Actonel 35mg 1 time a week. Lasix 40mg CD,
lododerm 5% patch, miralax 17 grams, potassium 20/MEQ 7.5ml TID, Aricept 5 mg QD
and 50mg of estraline and 10,000 units of Vitamin D a month.
Elizabeth is slowly experiencing a decline. Her weight is 190.6. She eats about 44% of
her pureed meals. She also has 4 supplements daily along with 4 oz. of ice cream after
her evening meal. Her sight continues to decline. She needs to be fed by staff because
of her failing eyesight.
She attends some activities including bingo and Mass. Presents were donated by
Home Instead Home Health and were given for her to open at Christmas.

Elizabeth is now 92 and is beginning to slow down a little. When Aging staff visits her
she is often asleep. When she is awake, she is in a great mood and laughs a lot. She
also likes to sing. This caused some problems with roommates until the home moved
her to be in a room with someone who is hard of hearing. The nursing home staff
indicated that she was doing well with no new orders from the doctor. Her daughter calls
her approximately twice a year. They usually have a short conversation that doesn't
seem to bother or excite Elizabeth. She is visited every 30-60 days by Aging staff and
the nursing home advises Aging staff of any needs or changes in orders.
It is the request of the Cumberland County Office of Aging that the agency retain
Guardianship of Person for Elizabeth Floyd, The Todd Home has demonstrated an
adequate quality of care for her medical and mental problems. We believe, therefore,
that Ms. Floyd should continue to reside in that nursing facility.
ESTATE
A guardianship account is maintained for Elizabeth at M&T Bank. In January 2013 her
Social Security increased to $1752,00 a month. The Sarah Todd Home receives
$1607.00 a month for her room, board, and care. The Cumberland County Aging and
Community Services receives $100 a month in guardianship fees as provided by the
court order. The balance of her checking account as of April 2013 was $2722.19.
Attached is a financial summary of Elizabeth Floyd's income and expenses as well as a
checking account register.
