Comes now Petitioner, through her attorney, Stephen J. Hogg, Esquire, and
respectfully asks this Court to issue an Order for changing Petitioner's name from
Sabaheta Oric, Petitioner's maiden name, to Sabaheta Vrtagic, her married name.
WHEREFORE, Petitioner prays this Court enter an Order directing required notice to
be given by Petitioner and schedule a hearing on this matter.

Identification Division Central Repository,and:

Police, upon receipt of this notice, shall include the change of name information in the Central Repository as
provided for in 18 PA C.S.,Chapter 91 (relating to criminal history record information).
