MANDATORY DUI SCHOOL /ALCOHOL HIGHWAY SAFETY SCHOOL
All defendants who are convicted of or plead guilty to a DUI in Pennsylvania are required to complete an Alcohol
Highway Safety School.
Below is a list of times and locations of Cumberland County Classes. Please CIRCLE YOUR CHOICE.
You will receive a letter telling you when and where to report for your class.
First Offenders
1. Mechanicsburg Night : 5 weeknight classes from 6:30 pm to 9:30 pm at the 1" Church of God Community
Center located at 201 E. Green Street, Mechanicsburg, PA
2. Carlisle Weekend : 2 classes : Saturday & Sunday from 9:00 am to 5:00 pm at the Carlisle Police Building
located at 240 Lincoln Street, Carlisle, PA
3. Mt. Holly Weekend : 2 classes : Saturday & Sunday from 9:00 am to 5:00 pm at the Citizens Fire Company
located at 100 Chestnut Street, Mt. Holly Springs, PA
4. Carlisle Day : 2 classes : 2 consecutive Saturdays from 8:00 am to 4:00 pm at the Carlisle Alliance Church
located at 237 E. North Street, Carlisle, PA
5. Lower Allen : 2 classes: Saturday & Sunday from 9:00 am to 5:00 pm at the Lower Allen Township Building
located at 2233 Gettysburg Road, Camp Hill, PA
Multiple Offenders
1. Weeknights : 5 weeknight classes from 6:30 pm to 9:30 pm at the Cumberland County Prison. 1101 Claremont
Rd., Carlisle, PA
2. Weekends : 2 classes : Saturday & Sunday from 9:00 am to 5:00 pm at the Carlisle Police Building located at
240 Lincoln Street, Carlisle, PA
PAYMENT INSTRUCTIONS
The court has ordered that you pay, in full, the costs of prosecution, fines and /or restitution in this case. If you have
previous dockets with active balances, you are also responsible for paying those balances in full.

WARNING: If you fail to comply with this payment agreement a warrant may be issued for your arrest and your case

You must comply with the following conditions:
1. Remain arrest free. Report any police contact to your Probation Officer immediately.
2. Obtain and maintain fulltime employment.
3. Remain on good behavior and refrain from behavior which may present a danger to yourself or others.
4. Provide proof of any valid prescriptions to your supervising officer if requested.
5. Submit to testing for drug /alcohol as directed by your Probation Officer. You may not use any synthetic, mood or
mind altering substances or any controlled substances without a valid prescription. Any attempt to substitute or
adulterate a urine specimen is a violation of this condition and may subject you to arrest on new charges.
6. Notify your probation officer before you change your physical or your mailing address.
7. Obtain a travel permit before leaving Pennsylvania.
8. Report as directed by your probation officer at least _ per week / month / quarter.
9. You may not have any weapons in your residence, vehicle, or person while on active supervision.
10. You must submit to a search of your person, vehicle, residence or lodging with or without a warrant.

DUI SPECIAL CONDITIONS
1. You must obtain and pay for a drug /alcohol evaluation and successfully comply with the recommendations of that
evaluation. You must sign a release of information for any treatment agency and Cumberland County Adult
Probation and Parole Department to exchange information for the duration of your sentence. It is your
responsibility to provide written verification of the completion of the requirements to your Probation /Parole
Officer. (Refer to attached sheet for list of approved providers)
2. You must pay for and successfully complete DUI School /Alcohol Highway Safety School as directed.
3. You may not possess or consume any alcoholic beverages or illegal drugs.
4. You must pay for and successfully complete a CRN evaluation.
