PETITION FOR CHANGE OF NAME
AND NOW, comes Laura L. Bouder, Petitioner, on behalf of the minor child, Brennen Michael
Nehf, by and through her attorney, Lindsay D. Baird, Esquire, and pursuant to 54 Pa.C.S.A. §702,
respectfully avers:
1. Laura L. Bouder, Petitioner, is the adult mother of the minor child and resides at 1936
Reservoir Drive, Carlisle, PA 17013.
2. Brennen Michael Nehf was born on April 12, 2011, is two years old and has resided all of his
life with your Petitioner.
3. The father is Michael Lee Nehf, Jr., and his last known residence was Cumberland County
Prison. Prior to that he lived with his mother, Mary Ott, at 353 East Burd Street,
Shippensburg, PA 17257 for one week. His whereabouts are currently unknown.
4. Ms. Bouder would like Brennen's last name to be the same as hers and his older brother's to
save Brennen confusion.
5. At the time Brennen was born, Ms. Bouder and the father were a couple intending to marry.
The father then relapsed in his heroin addiction. The father last saw Brennen when Brennen
was seven months old and has never contributed to his support.
6. There are no judgments of record against Brennen.
WHEREFORE, Petitioner requests this Honorable Court to:
a. Fix a hearing on this petition,
b. Direct that notice be given of the hearing by publication, and
c. Enter a decree changing the child's name from Brennen Michael Nehf to Brennen Michael
Bouder.
