Petitioners, John Henry Riddlesberger and James A. Smiley, Co- eckitors g4he ristaate

of May Kitzmiller Riddlesberger,by and through Mark A. Mateya, Esquire files the Petition to
Amend Petition for Grant of Letters and in support avers the following:
1. May Kitzmiller Riddlesberger, died testate on November 19, 2013.
2. Co-Executors John Henry Riddlesberger and James A. Smiley presented for
probate the duly executed Last Will and Testament and Death Certificate of May Kitzmiller
Riddlesberger on or about November 26, 2013,with the Cumberland County Register of Wills.
3. After the Petition for Grant of Letters, Death Certificate and Last Will and
Testament of May Kitzmiller Riddlesberger were presented to the Register of Wills of
Cumberland County, Pennsylvania for probate, it was discovered that the Social Security number
of the decedent was incorrect on the Death Certificate and Petition for Grant of Letters.
4. Corrected Death Certificates were issued correcting the Social Security number
for the decedent on 11/27/13.
5. The corrected Death Certificate was filed with the Register of Wills of
Cumberland County on December 4, 2013.
WHEREFORE, Petitioners respectfully request that this Honorable Court amend the

Petition for Grant of Letters to reflect the correct Social Security number of the decedent and to
issue Letters Testamentary to the Petitioners.
