PERSON :. �::.a c..a ;_ ,.,� -�, E. � � On September 25, 2008, Judge Edward Guido appointed the Cumb�rland C�� nty�0#i�e of Aging as temporary emergency Guardian of Person for Pauline Demler. Following the appointment, Pauline was admitted to Claremont Nursing and Rehabilitaton Center. On October 16, 2009 the Cumberland County Aging and Community Services was appointed permanent Guardian of Person. Ms. Demler's medical conditions at time of admission were coronary artery disease, peripherial vascular disease, diabetes milletus, hypercholesterol, hypertension, ischemic cardiomyopathy, gangrene right third toe, history of myocardia lnfarction (heart attack). She was later diagnosed with dementia with delirium, depression, allergic rhinitis, paranoia, hallucinations and chronic anemia. Medications she is receiving are insulin, amlodipine for blood pressure, aspirin for heart, isosobide for the cardiomyopathy, lisinopril for blood pressure, loratadine for allergies, plavix for CAS, risperiodine for behavioral issues, sertaline for depression, aricept for dementia, simvastatin for cholesterol, albuterol for wheezing and ibuprofen for pain as needed. Her insulin for diabetes was increased this year. Her Zoloft, given for mood disorder, was decreased because of her often flat effect but there was no change. In August 2012 she was taken to the ER for chest pains. She had been vomiting during the night and was very lethargic with minimal alertness. But her vitals were goo-lungs clear, sugar and fat levels were good. Her labs were good and an EKG was normal. It was decided that she had some kind of virus. She is seen on a routine basis by a psych consultant from Med Options because of her hallucinations and delusions. The medication has helped with this and she is currently not experiencing any. In January of this year she began displaying an increased confusion. She also became exit seeking so a roam alert was placed on her. She was later moved to the 3�d floor (the memory unit) because of this. In July 2013 she had a urinary tract infection but responded to medication. Pauline continues to require hands on assistance with her activities of daily living. She usually eats 100 % of her meals and her average weight is around 200 pounds. This year Priscilla and Wendy from Aging had a picnic for her on her birthday. They were outside and they brought her Kentucky Fried Chicken which is her favorite. She is also a great fan of Elvis. Aging staff visits with Pauline about every 30-60 days. Staff also attends the quarterly care plan meetings with nursing home staff from nursing, activities, social services, and a dietician. They also delivered Christmas presents to her and shopped for items that she needed during the year. It is the request of the Cumberland County Office of Aging that the agency retain Guardianship of Person for Pauline Demler. The Claremont Nursing and Rehabilitation Center has demonstrated an adequate quality of care for her medical and mental problems. We believe, therefore, that Ms. Demler should continue to reside in that nursing facility. ESTATE Following the appointment as guardian, Cumberland County Aging opened a guardianship checking account at Sovereign Bank where she did her banking. The funds in her personal accounts were transferred into the new guardianship account. The agency assisted Claremont Nursing and Rehabilitation Center, where she resides, filed for medical assistance which was approved. The agency applied and was named the representative payee for Ms. Demler with the Social Security Administration. Ms. Demler's Social Security, which is her only income is currently $681.00. During this year her main expense has been her portion of the payment toward the nursing care which is currently $$536.06 a month. The Cumberland County Aging receives a guardianship fee of$100.00 a month. This was approved by the court and included in the court order. The only other expenditures this year were for some clothing and Diet Pepsi. Christmas presents were donated by the Home Instead Secret Santa project. Ms. Demler had previously made pre-funeral arrangements so it was unnecessary for the Agency to do that. Attached is a financial summary of Pauline Demler's income and expenses.
