Plaintiff State Farm Mutual Automobile Insurance Company is an insurance carrier licensed to conduct business in the Commonwealth of Pennsylvania and having as one of its principal places of business the above - captioned address. 2. Virginia Garrett is an adult and on or about October 2, 2012, was the registered owner of a 2001 Subaru motor vehicle insured under a policy of insurance issued by State Farm Mutual Automobile Insurance Company. 3. Defendant Highmark Health Services is a business entity, authorized to transact business in the Commonwealth of Pennsylvania, and on or about October 2, 2012, was the registered owner of a 2012 Chevrolet motor vehicle bearing Pennsylvania License Plate: FLN8577. 4. Defendant Nancy Kellison is an adult residing at 131 Fairway Drive, Dillsburg, Pennsylvania 17019, and on or about October 2, 2012, was an employee and/or agent of Defendant Highmark, and operator of the aforesaid Chevrolet motor vehicle. 5. On or about October 2, 2012, while operating the aforesaid Chevrolet motor vehicle in the parking lot of the Lawn Pennsylvania Turnpike Service Station, located at milepost 258.8 Westbound between Exits 247 and 266 in Lawn, Pennsylvania, Defendant(s) failed to properly observe parking conditions and exited their parking space without proper clearance and struck the aforesaid Subaru motor vehicle as Plaintiff's insured traveled through the aforesaid parking lot with the right of way, causing extensive damage to Plaintiff's insured's motor vehicle in the amount of $918.12. 6. The aforesaid damage was a direct and proximate result of Defendant's negligence and /or carelessness. 7. Pursuant to the insurance policy herein mentioned, Plaintiff State Farm Mutual Automobile Insurance Company a/s /o Virginia Garrett has incurred and paid expense and other damages in the amount of $918.12. WHEREFORE, Plaintiff State Farm Mutual Automobile Insurance Company a/s /o Virginia Garrett demands judgment against the Defendants in the amount of $918.12 which includes Plaintiffs deductible, plus interest and costs of suit.
