Petitioner Jane Sheats is the natural daughter of Inez J. Mitchell, the alleged incapacitated person. 2. Petitioner Sarah Shaak is the natural daughter of Inez J. Mitchell, the alleged incapacitated person 3. Petitioner Kent Mitchell is the natural son of Inez J. Mitchell, the alleged incapacitated person. 4. Inez J. Mitchell, the alleged incapacitated person is 88 years of age and a widow. Her residence is located at 519 Penn Ayr Road, Camp Hill, Cumberland County, Pennsylvania, 17011. 5. On or about December 10, 2013, Inez J. Mitchell was admitted to Holy Spirit Hospital at 503 North 21sr Street, Camp Hill, Cumberland County, Pennsylvania 17011 for a medical admission pending a psychiatric evaluation. a. Since approximately March, 2013, Inez J. Mitchell has been suffering from auditory and visual hallucinations; b. On or about November 7, 2013, Inez J. Mitchell was diagnosed with Alzheimers. c. Despite the assistance of in-home care providers, Inez J. Mitchell is incapable of managing her medications and/or her physical care on a daily basis. 12. The proposed plenary co-guardians of the person and the estate of Inez J. Mitchell are: a. Jane Sheats, who resides at 4600 Brookridge Drive, Center Valley, Lehigh County, Pennsylvania 18034; b. Sarah Shaak, who resides at 3802 Chippenham Road, Mechanicsburg, Cumberland County, Pennsylvania 17050, and; c. Kent Mitchell, who resides at 309 Pinehurst Road, York, York County, Pennsylvania 17402. 13. The proposed plenary co-guardians of the estate and person have no interest adverse to the alleged incapacitated person. 14. No other court has ever assumed jurisdiction in any proceeding to determine the capacity of the alleged incapacitated person. 15. No other guardian has been appointed for the estate or person of the alleged incapacitated person. WHEREFORE, petitioner respectfully requests that this Honorable Court award a citation directed to Inez J. Mitchell, the alleged incapacitated person, and to such other persons as this Court may direct, to show cause why Inez J. Mitchell should not be adjudged a fully incapacitated person, and Jane Sheats, Sarah Shaak and Kent Mitchell should not be appointed plenary guardian of her person estate.
