Before me the undersigned notary public,this day personally appeared James Midgarden, Jr.,
429 Blacklatch Lane, Camp Hill, PA 17011, to me known, who being duly sworn according to
law, deposes and says:
I am the owner of a 1998 Dodge Dakota truck that was rear-ended by an uninsured motorist,
Christopher Brown on 23 July 2013. Christopher Brown was cited by the Lower Allen Township
police department for the accident for not having insurance. I obtained two estimates for repair. I
attempted unsuccessfully to call Christopher Brown several times to discuss payment for the
damages. I then sent a certified letter to Christopher Brown's address outlining the damages and
estimates. Shortly thereafter, Karen Brown, Christopher Brown's mother, called me indicating
that her son had left Pennsylvania for an unspecified period of time. I then initiated a Civil
Complaint with the County of Cumberland, Magisterial District Court in Camp Hill, PA. I was
awarded a judgment for $2853.76 on October 25th 2013. Christopher Brown did not appeal the
judgment, or respond to my correspondence. I then submitted the judgment to the Cumberland
County Prothonotary's office in Carlisle, PA. I am in the process of submitting form DL-201,
Certification of Motor Vehicle Judgment to the Pennsylvania Department of Transportation.
I hereby state that the above information is true to the best of my knowledge. I also confirm that
the information here is also accurate and complete, and relevant information has not been
omitted.
