Petitioner is the mother of the minor child, Samantha Elizabeth Rittner, now age 16, having been born on 3 October 1997. Petitioner files this Petition on behalf of her said daughter. 3. Petitioner and Samantha Elizabeth Rittner had resided at the 3708 Swindon Lane, Mechanicsburg address for five continuous years prior to the date of filing this Petition. 4. Petitioner asks this court to change the name of her daughter from Samantha Elizabeth Rittner to Samantha Elizabeth Rittner Renee. 5. Petitioner is contemporaneously filing a Petition, in her own behalf, to change her name from Susan Renee Rittner to Susan Pust Renee. The reasons for her requested change of name are set forth in that Petition. 6.
