Pauline Cover's guardianship proceedings. We have enclosed our Petition and the Preliminary Order of
Court as well as an Affidavit of Service and Consent to Serve as Guardian of the Estate for Pauline Cover.
At your earliest convenience, please serve Pauline Cover in person with a copy of the Preliminary Order
and Important Notice with Citation, returning to us, a signed affidavit of service. Also kindly sign and

I traveled to Golden Living West Shore to visit with Pauline. When I arrived she was in
the dining area of her unit. She was sitting at a table, prior to my visit she had been
yelling and causing a commotion,but when I sat down beside her she seemed calm, but
nervous, wanting me to call her family because she was scared. She told me to call her
daughter's Brenda Hall and Kathy Wisner. I told her that she had nothing to be afraid
about, she said that those girls don't listen to her, I told her that they did and that they
would take good care of her. I told her that I had another friend who had been in here
and that they took very good care of her and I was sure that they would do the same for
her. She asked me to call her family so I said that I would. I told her that they were just
getting ready to have Root Beer Floats so she should just sit there and wait and that the
girls would take good care of her. She said you are very nice, would I stay with her
because she was afraid, I told her that the girls would see that she was fine and I would
be back another time to visit, she said promise I said yes. GME

Melanie and I traveled to the Cumberland County Courthouse in Carlisle for the hearing
regarding the guardianship of Pauline Cover.
Testimony was heard however when Pauline's Daughter, Kathy,took the stand and stated some
information the Judge decided to continue the hearing to give Kathy an opportunity to seek
counsel. The Judge gave her the phone number to contact the Legal Services for those who do
not have funds to pay for attorney's. Those present were somewhat confused at continuing the
hearing for this reason. Golden Living had to ask for an extension for the MA because of this;
the Judge set May 3i at li:oo for the next hearing. CES

Melanie and I traveled to The Cumberland County Courthouse in Carlisle for the second hearing
regarding the guardianship of Pauline Cover.
Kathy did not have an attorney; she told the Judge she did not have one because they said they
do not provide the type of legal service that she needs in this case. Kathy spoke for herself.
The Judge did have more questions for Attorney Jackson he pointed out the Guardian has the
right to advise the previous Agent under a POA they may not serve.
The order did state that the POA which Kathy held is no longer valid with the issuance of this
order.
Kathy made it very clear that she did not want to talk to anyone from Golden Living or from
Keystone Guardianship Services; as a matter of fact when she entered I smiled and said hello
she glared at me and asked me what was so funny. Attorney Jackson offered her my card she
said she didn't want to talk to me and that if I had something to say to her I should tell my
attorney and he will tell her, she added after all isn't that the way it is when one has an attorney.
She threw my card in the waste can and even later pulled it our and tore it in many pieces in
front of the attorney.
Kathy did tell the judge she will send the information regarding the insurance to Golden Living
and that they should stop sending her the bills. CES
This same day Melanie and I went to the Social Security Office in Carlisle to become the rep
payee for Pauline Cover. We did not have an appointment but they were willing to see us if we
could come back at i:i5 PM. CES

sign paperwork.
Prepared and fa�ced the direct deposit form as promised to Mrs. Dengler at the CC
SS office.
Completed and delivered to post office; the request for mail forwarding for anything
for Pauline going to 35 Scarsdale Dr.
I completed a property search on 35 Scarsdale Dr. and it does reflect that the
properry is now in Kathleen Wisner's name from a sale of$1 on 3/6/2012, with an assessed
value of$i64,lo0.00. MLP

TCF Brenda Hall to advise that Pauline is in a decline right now. She is not eating or
drinking. Brenda did sign the necessary end of life paperwork; and they have Hospice
lined up. Brenda was wondering if we located anything on a burial and I advised that we
did not, however we have really received nothing yet. We had initiated the mail forward on
6/3, but it can take up to 9 business days for it to go into effect and will only forward what
is in Pauline's name. I told her I would review and see if we can locate anything, but I got
the impression at the hearing that Kathleen had not set up a burial reserve. She said to call
her back on her cell of 4i8-i584. MLP

Mel received a phone call from Brenda that Pauline passed away on today's date, June 1�,
2oi3, and that Brenda did not know what to do from this point. She also said that she
talked to Neil Funeral Home and that they were going to call us. CES

Time:
TCT Brenda Hall and explained that the law says that when the Ward dies,the guardian
dies at the same time, and I also went on to tell her with our recent appointment, we did
not receive anything from Prudential and that we agree with Brenda that at the hearing it
was stated that there is a $i,800 Prudential policy that was in force Pauline's burial.
I also told Brenda in my opinion I would think that the daughter Kathy would now be in
charge since she is the next of kin. Brenda said that Neil Funeral Home said that Kathy
needs to sign for the cremation as well and that they are meeting at i:oo pm.
I also called Kevin of Neil Funeral Home advising him that we have nothing regarding
Pauline Cover's burial and explained that we just recently became the court appointed
guardian and that we have not yet received anything from Prudential.
I told him at the hearing it was said that there is a Prudential Life Insurance policy which is
meant to be used for her burial and also that Kathy is living in the house that was owned by
the deceased.
He said that they will need to come up with some arrangement or they will not provide
services for her. CES

We have not uncovered any assets for Pauline; the mail forward did not bring any asset
information. We noticed the real estate was transferred for a dollar on March 6, 2012.
We are not in receipt of any notice of charge accounts or loans that Cathy said in the hearing were
outstanding.
As far as we can tell there are not estate assets to probate.
I think it goes without my saying, if we serve as Executor we need to be guaranteed payment for our
services, since we have already put much time and expense into this case without payment far the
same.
Thank you, we will advise you if we receive any notices of assets ar debt of the Pauline. If you
become aware of the person to whom we should forward the mail, please let us know; otherwise it
will accumulate in our office.

We just found out yesterday that Pauline Cover passed away over the week-end. So the
guardianship ended with her death. Now we are starting to think about another course of action.
We realize it has been a relatively short period of time since your appointment as guardian but have
you been able to uncover any assets? Would you be willing to serve as Executor if it was
determined that there were recoverable assets and a probate estate was opened?

Attached you will find a copy of the check we received through the mail forwarding.
Interestingly when we pulled the SS statement to send to you we noticed there are two different
statements with two different amounts and SS#s; apparently she was receiving a survivor's benefit. I
thought when one dies the remaining just receives the higher amount; not both checks.
We are stopping the mail forward because we are no longer authorized to act.
This may lead to a real mess for"Cathy"! I am not an attorney but maybe a lien can be placed on the real
estate by Golden Living or watch for an estate to be settled and take against the estate; again just my
thought on what I might do if I would be owed money.

represent Golden Living and asked if we would be a part of a telephone conference
regarding the MA application. I discussed same with Melanie and she said she would do

questions regarding the application for Medical Assistance. I told him that we have a time
for a hearing on the lith regarding this situation and also that Pauline is deceased. He was
not aware of that but I did tell him if he wants, I will have Melanie call to see if there is
anything she can assist him with. CES

I told him that we have proof of her SS and the UGI pension, but we have to date, not
received any bank statements to know anything about the resources. I asked if he thought
that the hearing would still be in 2 days. He said that he thinks that it will still happen. It
appears that a decision will just have to be made that everyone attempted to get the
resources,but were unable to.
He asked if I could fax him the proof of income and gave me fa�c # (8i4-86�-i914).

Also, I called the Neil Funeral Home and was told that Kathy paid cash for the
funeral because the Prudential Policy had her listed as the beneficiary; so apparently
she redeemed it and paid cash so there is nothing to obtain from the funeral home.

Estate.
