It is averred that Defendant was indebted to HSBC BANK NEVADA, N.A. / ORCHARD BANK on October 31, 2001 with account number * * * * * * * * * ** *8425 (hereafter referred to as "Account "). A copy of the account history is attached here to and collectively marked as Exhibit "A." 4. By using the Account, Defendant agreed to repay any incurred balances and/or charges made to the Account pursuant to the terms and conditions governing said Account.
