Petitioner, Jodie L. Heider, respectfully states:
1. Petitioner is Jodie L. Heider, an adult individual who resides at 6177 Haymarket Way,
Mechanicsburg, PA 17050.
2. The minor child, CASEY E. STEVENS, was born on May 2, 2000, and is currently
thirteen (13) years old.
3. Petitioner is the minor child's other.
4. The minor child lives at 6177 Haymarket Way, Mechanicsburg, PA 17050, with the
proposed guardians, Jodie L. Heider and Timothy J. Heider.
5. Other individuals living with Petitioner, the proposed co-guardian and the minor child are:
Paige Williams, 17, friend; David Williams, 15, friend; Jonas Heider, 6, friend; Pauline
Heider, 62, friend. The minor child, CASEY E. STEVENS, is enrolled in school as
follows: Grade - 8th grade; School - good hope middle school; School district-
cumberland valley.
6. The mother of the minor child is Deborah L. Keeley, whose whereabouts are unknown.

8. The father of the child is unknown.
9. The child is in need of a guardian because the mother's whereabouts are unknown; the
identity of the child's father is unknown; the child's school is requiring legal authority to
deal directly with the proposed guardian regarding the child's education; a physician or
other medical provider is requiring that the proposed guardian have legal authority to
obtain medical treatment for the child; and the proposed guardian wishes to have
general legal authority to be able to protect and provide for the child in any
circumstances which may arise in the future.
10. The consent of the child's mother is not attached because her present whereabouts are
unknown.

11. The consent of the child's father is not attached because the identity of the child's father
is not known.
12. Petitioner, age 36 years, and Timothy J. Heider, age 38, are the proposed Guardians of
the Person of the minor child.
13. The proposed co-guardian, Timothy J. Heider, lives with Petitioner at 6177 Haymarket
Way, Mechanicsburg, PA 17050, and is the friend of the child. He is also Petitioner's
husband.
14. Petitioner believes that she and Timothy J. Heider are qualified and suitable to be
appointed as Guardians of the Person of the minor child.
15. The consents of the proposed Guardians are attached to this petition.
WHEREFORE, Petitioner, Jodie L. Heider, nominates Timothy J. Heider and herself as
Guardians of the Person of CASEY E. STEVENS, and requests this Honorable Court to make
such appointment pursuant to Section 5111 of the Probate, Estates and Fiduciaries Code, 20
Pa.C.S.A. § 5111.

Verification
I, Jodie L. Heider, Petitioner, verify that the facts stated in the foregoing Petition for
Appointment of a Guardian of the Person of a Minor under the age of fourteen (14) years are
true and correct to the best of my knowledge, information and belief. Petitioner understands
that false statements therein are subject to the penalties of 18 Pa.C.S.A. §4904 relating to
unsworn falsification to authorities.
