CONSULTATION NOTE
RE: Diana Fenton Pate
Diana Fenton Pate suffers from fronto-temporal dementia. Her neurological condition has
deteriorated rather rapidly. At present the patient is unable to speak. She has no lower extremity
strength. Her right arm is clenched and assuming flexion posturing with increased spasticity in
her arms, right greater than left.
The patient needs 24 hour supervision. Her husband is using Hospice as well as Homestead
Nursing to help her with her activities of daily living. Her weight is stable. The patient has no
difficulty with swallowing and denies any choking. She experienced 14 seizures since her last
office visit, and is responding to her medication, Lorazepam.
At present, Diana Pate is completely debilitated to the point that she is completely dependent on
others for dressing, eating and transferring. She is non-communicative. The patient has
developed seizures which are responding to Lorazepam. Palliative care is suggested.
Support to her friend was given.
