PETITION FOR CHANGE OF NAME
TO THE HONORABLE, THE PRESIDING JUDGE OF SAID COURT.
The petition of Mary and William Kelly on behalf of their adopted daughter, Yi
Hai Qian, filed pursuant to 54 Pa. Cons. Stat. Section 701 et seq., respectfully sets forth
that
1. Your Petitioners' Mary and William Kelly (hereinafter referred to as
"Petitioners") daughter, Yi Hai Qian, is a 4 year old, and was born April 18,
2000 in Hunan Province, China.
2. Yi Hai Qian is a current resident of Camp Hill, Cumberland County,
Pennsylvania, and has been since April 27, 2001. Since April 27, 2001, Yi
Hai Qian has resided at 4 East Gate Drive, Camp Hill, P A 17011
3. Yi Hai Qian's birth parents are unknown.
4. Yi Hai Qian was adopted in Hunan Province, China on April 16, 2001. See
attached photocopy of Birth Certificate and Adoption Certificate provided by
the Provincial Government of Hunan China.
5. The Petitioners desire to have their daughter take her adopted family name.
6. Petitioners desire to have their daughter known as Faith Hai Mary Kelly.
7. There are no judgments or decrees of like character against Yi Hai Qian.
WHEREFORE, your Petitioners respectfully request this Honorable Court to
enter a decree changing their daughter's name from Yi Hai Qian to Faith Hai Mary Kelly.
