LI-OS--ll1- ORDER OF COURT FINAL DECREE tA AND NOW, this ~ of ~ ,2005, upon consideration of the attached a hearing held thereon, this Court finds, by clear and Petition and evidence received at convincing evidence, that ELIZABETH ANN ELLIS is adjudged a totally incapacitated person. The Court finds that ELIZABETH ANN ELLIS is in an unresponsive condition as a result of a motor vehicle accident, a condition that totally impairs her capacity to care for herself, to receive and evaluate information effectively and to make and communicate decisions concerning her management of financial affairs or to meet essential requirements for her physical health and safety. Petitioner, JACQUELINE A. ELLIS, is hereby appointed plenary permanent guardian of the PERSON of ELIZABETH ANN ELLIS. Petitioner shall in that capacity file a report on the social, medical and other relevant conditions as required by 20 Pa.C.S.A. S 5521 (c). Petitioner is also hereby appointed plenary permanent guardian of the EST ATE of ELIZABETH ANN ELLIS. Petitioner shall not be required to post a Court approved bond, but Petitioner shall file a report in compliance with 20 Pa.C.S.A. S 5521(c). The guardian of the PERSON shall have the authority and responsibility to decide where ELIZABETH ANN ELLIS shall live and how meals, personal care, transportation and recreation will be provided. The guardian of the personal shall also have the authority to authorize and consent to medical treatment and surgical procedures necessary for the well being of ELIZABETH ANN ELLIS. The guardian of the ESTATE shall have the authority and responsibility to manage and use ELIZABETH ANN ELLIS' property and financial resources for her benefit. The guardian of the estate shall have authority over all issues relating to her cash, checks, and any bank or savings accounts held in her name, her personal property, her entitlement to any governmental and nongovernmental benefit plans, federal, state, and local taxes, claims made or to be made on behalf of her or against her, the execution of any documents necessary and related to the above, and the entry into contracts affecting her as well as the payment of reasonable compensation or costs to provide services to her. The aforementioned judicial determinations have taken into consideration the matters required by 20 Pa.C.S.A.S5512.1.
