AND NOW, Jacqueline A. Ellis, being duly sworn according to law, does depose and
state:
1. That she is a competent adult and mother of Elizabeth A. Ellis, the alleged
incapacitated person in the above-captioned action.
2. That she served a copy of the Petition for Appointment of Emergency Guardian of the
Estate and Person in Accordance with 20 Pa.C.S.A. 9 5513, and read the Important Notice
Citation to Elizabeth A. Ellis, on February 'i?, 2005, by hand-delivery to her at HealthSouth
Rehabilitation, Mechanicsburg, Cumberland County, Pennsylvania.
I verify that the statements made in this affidavit are true and correct. I understand that
false statements herein are made subject to the penalties of 18 Pa.C.S.A. 94904, relating to
unsworn falsification to authorities.
