North Middleton Authority, North Middleton Township, County of Cumberland, Commonwealth of
Pennsylvania by its solicitor, Martson Deardorff Williams Otto Gilroy & Faller, hereby files its
claim for sewer services against the real estate hereinafter described and sets forth its claim as
follows:
STATEMENT OF CLAIM
1
The name of the municipality by which this claim is filed is North Middleton Authority, a municipal
authority of the Commonwealth of Pennsylvania, located in the County of Cumberland and
organized pursuant to ordinances of North Middleton Township.

The name of the owner of the property against which this lien is filed is Johnnie B. Pond of 50
Bonny Brook Road, Lot #17, Carlisle, Pennsylvania 17013.

A description of the property against which this Lien is filed is attached hereto and marked as
Exhibit "A" with an address of 119 Brookwood Drive, Carlisle, Pennsylvania 17013.

The authorities under and by virtue of which this Lien is filed is the Municipal Claims and Liens Act
of May 16, 1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq.

The lien is claimed for unpaid water and sewer user fees against the above - described property in the
amount of $305.93, plus any additional fees incurred thereafter.

Interest is claimed on the above - mentioned fees until paid in full.

receipt whereof is hereby acknowledged, the said Grantors do hereby grant and convey
in fee simple to said Grantee, his heirs and assigns,
ALL THAT CERTAIN parcel of land and premises situate, lying and being in North
Middleton Township, Cumberland County, Pennsylvania, together with the
improvements erected thereon more particularly bounded and described in accordance
with a survey made by Thomas A. Neff, Registered Surveyor, on February 21, 1966, as
follows, to wit:
BEGINNING at a stake on the northerly side of Brookwood Drive (formerly known as
Schlusser Avenue), said stake being located and referenced at a distance of 200 feet
measured along said Brookwood Drive in an easterly direction from the easterly side of
Wood Avenue, said stake being at the division line between Lots Nos. 33 and 32
(erroneously referred to as Lots 33 and 43 in prior deed) on the Plan of Lots hereinafter
mentioned, at lands now or formerly of Arthur G. Upperman; thence along the division
line between said Lots Nos. 33 and 32 on the Plan of Lots, and lands now or formerly of
Arthur G. Upperman, North 15 degrees East, a distance of 110 feet to a stake; thence
along lands now or formerly of George Case, South 75 degrees East, a distance of 100
feet to a stake at the dividing line between Lots Nos. 31 and 30, at a corner of lands
now or formerly of Harold F. Hall, Jr.; thence along said dividing line between Lots Nos.
31 and 30, and lands now or formerly of Harold F. Hall, Jr., South 15 degrees West, a
distance of 110 feet to a stake on the northerly side of Brookwood Drive aforesaid;
thence along said northerly side of Brookwood Drive, North 75 degrees West, a
distance of 100 feet to a stake on the same at the division line between Lots Nos. 32
and 33, the place of BEGINNING.

BEING composed of all of those 2 certain lots of ground known as Lots Nos. 31 an 32
as shown on a Plan of Lots known as "Valley View Extension" as the same is recorded
in the Office of the Recorder of Deeds in and for Cumberland County, Pennsylvania in
Plan Book 6, Page 6.
HAVING THEREON ERECTED a dwelling house known and numbered as 119
Brookwood Avenue, Pennsylvania.
UNDER AND SUBJECT to easements, rights -of -way, oil and gas leases, restrictions,
reservations, exceptions, agreements, and coal and mining rights as set forth in prior
instruments of record.
BEING the same premises which Shirley A. Richards, single woman, by Deed dated
April 25, 2005 and recorded April 26, 2005 in the Office of the Recorder of Deeds in and
for Cumberland County in Deed Book 268, Page 2840, granted and conveyed unto
Coleen M. Cotton, Grantor herein.
AND the said Grantor hereby warrants specially the property herein conveyed.
