consideration of the annexed Petition for Review Hearing and Substitution of the Guardian of the
Person and Estate of Steven Joshua Bergey, it is hereby ORDERED and DECREED that:
1. A citation is awarded, directed to Steven Joshua Bergey to show cause why a Review
Hearing should not be held to address the current guardian's failure of duties.

2. The hearing on the annexed Petition shall be held on the �� day of
, 2014, at e�'.c7p o'clock� M. (Prevailing Time). The hearing will be held
before Judge �:.��G in Courtroom�c�-- , located in the Cumberland County
Courthouse, in Carlisle, Pennsylvania.
3. Petitioner shall cause to be served (by personal service) the Citation with Notice and
Petition, pursuant to the provisions of 20 Pa.C.S.A. §5511(a) upon the incapacitated person at
least twenty (20) days prior to the court hearing. The contents and terms of the Citation with
Notice and Petition shall be read and explained to the maximum extent possible in language and
terms the incapacitated person is most likely to understand in accordance with the provisions of
20 Pa.C.S.A. §5511(a). An affidavit of service containing specific averments as to the above
requirements shall be presented at the beginning of the court hearing.
4. At least twenty (20) days notice of the court hearing together with a copy of the
petition shall be given personally or by certified mail to all persons who are not under any legal
disability and who would be entitled to share in the estate of the incapacitated person if he died
intestate, to the Guardian, Jessica Simes-Bergey, to the trustee of the special needs trust,
Matthew Menges, Esq., and to the person or institution providing residential services to the
incapacitated person, in accordance with the provisions of 20 Pa.C.S.A. §5511(a).
5. The guardian, Jessica Simes-Bergey, shall submit an annual report to the Court of her
management of the incapacitated person's person and estate by the time of the hearing.
6. The incapacitated person is EXCUSED from attending the Court hearing due to his
incapacity.
