FIRST COUNT
1. Plaintiff is American Express Bank, FSB ( "Plaintiff'), a federal savings bank, organized
under the laws of the United States of America with a principal place of business in Salt
Lake City, Utah.
2. Defendant(s) is /are CRYSTAL HOCKENBERRY ( "Defendant "), an adult individual
residing at 104 E NORTH ST, CARLISLE, PA 17013.
3. At the special instance and request of Defendant, Plaintiff issued a credit account
( "Account ") to Defendant.
4. The Account number ends in 2007.
5. Defendant received, accepted and used the account by making purchases, balance transfers,
and/or cash advances.
6. The account is in default due to Defendant's failure to make timely payments.
7. Although demand has been made, Defendant has failed to make payment of the amount
due.
8. The amount due as of this date is $11,362.84.
WHEREFORE, Plaintiff demands judgment in favor of Plaintiff and against Defendant(s)
for the sum of $11,362.84 and requests this Court award costs to the extent permitted by
applicable law.

How Rates and Fees Work
Calculating The Annual Percentage Rate (APR) for variable rates is determined by adding an amount (margin)

Returned Check $38 if you use your card to cash a check at one of our approved locations and the check is returned
unpaid. We will also charge you the unpaid amount.

Paying Interest: Your due date is at least 25 days after the close of each billing period. We will not charge you interest on your purchases if you pay the
