AND NOW, thi s 6 th day of Februar� 2 014,�o�e�L. '
Heider and Timothy J. Heider are appointed guardians of the
person of Casey E. Stevens, a minor, and as such are authorized:
1. To exercise physical and legal custody of the
minor.
2 . To arrange, choose, and consent to medical,
dental, surgical, or other healthcare treatment and procedures,
including emergency situations .
3 . To enroll the minor in school, give consents,
or make decisions regarding school activities, educational
placements or curricula and other matters regarding the child' s
education and school involvement with the understanding that the
Court will be contacting the school district to facilitate an
IEP.
4 . To sign or execute any contract or other
do�ument to provide support, health, education, or general
welfare of the minor.
5 . To take those steps that are reasonable,
necessary, and proper to provide for the support, health,
education, �and welfare of the minor.
The guardian shall promptly notify this court if
at any time they cannot act or no longer wish to act as guardians
for the minor.
This order, while temporary in nature, shall not

expire until further Order of Court. By separate order the Court
shall appoint a guardian ad litem for Casey E. Stevens and will
require a report from that guardian and/or a hearing based on the
guardian ad litem' s report to determine if this order shall be
made permanent, and, if so, what other additional terms or
requirements are necessary in the best interests of Case�r E.
Stevens.
