Petition for Appointment of a Permanent Plenary Guardian of the Person and Estate of
Raymond S. Witt pursuant to 20 P.S. Section 5511, and following hearing at which
evidence was presented by the Petitioner in support of the Petition, and,based upon the
foregoing, it is hereby ORDERED and DECREED:
1. That Raymond S. Witt is an incapacitated person in that he suffers from
severe cognitive deficit that prevents him from receiving and evaluating information
effectively and prevents him from formulating and communicating decisions to such a
significant extent that he is unable to manage his financial affairs or meet essential
requirements for his physical health and safety on a daily basis, and is adjudicated as
such.
2. That the Court appoint Gordon Witt and Rosalind C. Jones as Permanent
Plenary Guardians of the Person and Estate of Raymond S. Witt. No bond shall be
required of them.

3. That the Guardians shall file on a timely basis with the Court all reports
and accounts required by statute.
4. That the existing power of attorney be revoked and become null and void.
