North Middleton Authority, North Middleton Township, County of Cumberland, Commonwealth of
Pennsylvania by its solicitor, Martson Deardorff Williams Otto Gilroy & Faller, hereby files its
claim for sewer services against the real estate hereinafter described and sets forth its claim as
follows:
STATEMENT OF CLAIM
1
The name of the municipality by which this claim is filed is North Middleton Authority, a municipal
authority of the Commonwealth of Pennsylvania, located in the County of Cumberland and
organized pursuant to ordinances of North Middleton Township.

The names of the owners of the property against which this lien are filed are Charles R. Haines, 111,
and Alissa M. Haines, of 26 Runyon Drive, Trenton, NJ 0610 -4133.

A description of the property against which this Lien is filed is attached hereto and marked as
Exhibit "A" with an address of 419 Eisenhower Drive, Carlisle, Pennsylvania 17013.

The authorities under and by virtue of which this Lien is filed is the Municipal Claims and Liens Act
of May 16, 1923, P.L. 207, as amended and supplemented, 53 P.S. 7101 et seq.

The lien is claimed for unpaid water and sewer user fees against the above - described property in the
amount of $309.56, plus any additional fees incurred thereafter.

Interest is claimed on the above - mentioned fees until paid in full.

Made the 15 day of June in the year of Two Thousand Ten (2010)
Between KEYSTONE ARMS ASSOCIATES, LLC, a Pennsylvania limited liability
company, of the County of Lancaster and Commonwealth of Pennsylvania, (hereinafter called the
Grantor), of the one part,
AND
CHARLES IL HAINES, III, and ALISSA M. HAINES, husband and wife, of the County of
Cumberland and Commonwealth of Pennsylvania,
(hereinafter called the Grantee), of the other part,
Witnesseth That in consideration of ONE HUNDRED SEVENTY THOUSAND
and 00 /100 DOLLARS ($170,000.00) in hand paid, the receipt whereof is hereby
acknowledged, the said Grantor does hereby grant and convey unto the said Grantees, their heirs

described as follows:
BEGINNING at a point on the southern right -of -way line of Eisenhower Drive at the
northeast corner of Lot 48; thence along said right -of -way line N 54 °33' 19" E a distance of 22.00
feet to a point at the northwest corner of Lot 50; thence along said lot S 35 °26'41" E a distance of
166.21 feet to a point at the northeast corner of Lot 65; thence along said lot S 54 °33' 19" W a
distance of 22.00 feet to a point at the southeast corner of Lot 48; thence along said lot
N 35 °26'41" W a distance of 166.21 feet to a point, the point of BEGINNING.
CONTAINING 3,657 square feet and being Lot 49 of Keystone Arms.

BEING PART OF THE SAME PREMISES which Dickinson College, a nonprofit
corporation, by deed dated September 17, 2004 and recorded in the Office of the Recorder of
Deeds in and for Cumberland County, Pennsylvania, in Book 265, Page 2860, granted and
conveyed unto Keystone Arms Associates, LLC, a Pennsylvania limited liability company, its
successors and assigns.
ALSO BEING the same premises identified as Unit 49 in the Declaration of Covenants
and Restrictions for Keystone Aims, a planned community recorded in Book 725, Page 2457 as
amended by a First Amendment to Declaration of Covenants and Restrictions for Keystone
Arms, a planned community recorded in Book 729, Page 478 ( "Declaration ").
Under and subject to the Declaration and to all covenants, conditions, encumbrances,
easements and restrictions of record.
