On or about September 5, 2013, Capital BlueCross ("Capital") filed a proof of claim
against the Estate of Steven R. Smith. Since that time Capital has received payment in full for
that claim. This is a formal letter requesting that you withdraw Capital's claim against the
estate. Enclosed please find our check in the amount of$10.00, representing the fee to file this
Satisfaction of the Claim.
Please time-stamp the copy of this letter and return it to me in the enclosed, self-
addressed, stamped envelope. By copy of this letter we are notifying the executors and
attorney for the Estate of the withdrawal of this claim.
