Petitioner, Anna Marie McEntee, is an adult individual residing at 401 5th Street, Summerdale, Cumberland County, Pennsylvania. 2. Petitioner has resided at 401 5th Street, Summerdale, Cumberland County, Pennsylvania, for approximately six (6) months. 3. Petitioner's date of birth is June 12, 1934. 4. Petitioner has been a resident of Cumberland County, Pennsylvania for at least six (6) months preceding the filing of this Petition. 5. Petitioner desires to change her first name and middle name from Anna Marie to Tirzah Anna. 6. Petitioner believes this IS her God-given name, and for religious reasons desires to change her name. 7. Petitioner has been using the first name of Tirzah for her religious mail and telephone bill for approximately the last one-and-one-half years. SAIDIS SHUFF, FLOWER & LINDSAY A1TQRNEYS-AT.LAW 26 W. High Street Carlisle, PA 8. Petitioner now desires to legally take the first name and middle name of Tirzah Anna, thereby legally changing her name to Tirzah Anna McEntee. 9. Petitioner does not wish to change her name for any illegal purpose or to avoid any obligation(s). Petitioner has been fingerprinted by the Pennsylvania State Police and her fingerprint card is attached hereto as Exhibit U AU and by reference incorporated herein. 10. Petitioner has never been convicted of a felony. 11. Petitioner's Social Security number is 204-26-8089. WHEREFORE, Petitioner, Anna Marie McEntee, respectfully requests this Honorable Court issue an Order legally changing her name from Anna Marie McEntee to Tirzah Anna McEntee.
