"Petition to Transfer Ownership of a Motorcycle by Court Order"
We are petitioning the court to obtain a title for the 1996 Honda RVF 400, vehicle number NC35-
1100656.
We purchased this motorcycle in August, 1998 from Miyuki Performance Cycle Imports in Morgan
Hill, CA. The motorcycle is Japanese and did not come with a title.
We have had several customers express an interest in purchasing the motorcycle; therefore, we need to
obtain a title for it.
The Pennsylvania Department of Transportation has rejected our title application because there is no
original title.
Therefore, we are petitioning the court to grant ownership so that we can obtain a title.

"Petition to obtain an extension on court hearing"
I am petitioning the court to obtain an extension on my September 25, 2014 court hearing on the
Involuntary Transfer of Ownership of a Vehicle.
I have obtained the necessary records from PennDOT for the motorcycle and applied for the same
documentation from the California Department of Transportation. I spoke directly with the California
Depaitilient of Transportation and filled out the paperwork that was recommended. However, three
weeks later I have received notice that the incorrect form was filled out. I have completed the correct
form and returned it.
I am not sure if the information will arrive in time for my next hearing; therefore, I am asking for an
extension.

I am petitioning the court to obtain an extension on my December 18, 2014 court hearing on the

I have followed the procedures #1 and #2 on the Fact Sheet for the Involuntary Transfer of Ownership

consecutive weeks will not be completed in time for the December 18, 2014 court hearing. Therefore, I
am asking for an extension.
