Petitioners, Mayur J. Thakker and Maitri R. Trivedi, husband and wife, reside at 116 November Drive Apt. #3 in Camp Hill, Cumberland County, Pennsylvania 17011. 2. On December 20, 2013, Petitioners filed an application for marriage in Cumberland County, Pennsylvania. (See attached Marriage Record Exhibit "A"). 3. On December 23, 2013, Petitioners were issued a Marriage License No 1128 (See attached Cumberland County License Certification Exhibit"B"). 4. On December 31, 2013, Petitioners were married by Magisterial District Justice Mark Martin in Mechanicsburg, Cumberland County, Pennsylvania . 5. Upon receipt of-the Certification of Excerpts from Marriage License Record, Petitioners noticed incorrect information regarding the Groom's parents' names, namely, in the section of the Marriage Application entitled "Given and Sumame of [Groom's] Father", the Groom mistakenly inputted his own name rather than his father's given and surname; i.e., "THAKKER, JASVANTKUMAR, SHANKARLAL" and in the section of the Marriage Application section entitled "Maiden Name of [Groom's] Mother", Groom mistakenly inputted "SHARDABEN"in the Groom's mother's maiden name rather than only"THAKKER'. WHEREFORE, the Petitioners pray this Honorable Court to direct Clerk of the Orphans' Court to correct and amend Marriage License Docket, Vol. 2013, Page 112 per Marriage License No. 1128 to reflect the Groom's father as "THAKKER, JASVANTKUMAR, SHANKARLAL" [Last, First, Middle Name] and the maiden name of the Groom's mother as "THAKKER".
