Defendant, GREGORY S MELVIN, is an adult individual with last known address of 819 W TRINDLE RD, MECHANICSBURG PA 17055. 3. It is averred that Defendant was indebted to HSBC BANK NEVADA, N.A. / CAPITAL ONE BANK (USA), N.A / AMERITECH on August 17, 2003 with account number * * * * * * * * * ** *3253 (hereafter referred to as "Account "). A copy of the account history is attached here to and collectively marked as Exhibit "A." 4. By using the Account, Defendant agreed to repay any incurred balances and/or charges made to the Account pursuant to the terms and conditions governing said Account.
