PETITION TO TRANSFER CASE
1. The Petitioner is G. Patrick O'Connor, an adult individual residing at 503 Penn Ayr
Road, Camp Hill, Cumberland County, Pennsylvania 17011.
2. John J. O'Connor, an incapacitated person,resides at 103 Deerview Drive,Nevwille,
Cumberland County, Pennsylvania 17241.
3. The Petitioner was appointed guardian of the estate of John J. O'Connor on December
6, 2006,by the Honorable Todd A. Hoover, President Judge of the Court of Common
Pleas of Dauphin County, Pennsylvania.
4. The Honorable Todd A. Hoover has directed the guardian to take the necessary steps
to have this case transferred to Cumberland County by Order dated the 14th day of
June, 2013. A true and correct copy of said Order to attached hereto as Exhibit A.
WHEREFORE, the Petitioner respectfully requests that this Honorable Court
transfer this case to the Orphans' Court of Cumberland County, Pennsylvania.
