the Petition of Dawn M. Smoker, Guardian of Emily A. Smoker and following a hearing held on
this date at which hearing evidence was presented by the Petitioner in support of the petition and
based on the finding of facts contained in the accompanying opinion, the Court finds that it is
medically necessary that a hysterectomy be performed upon Emily A. Smoker and hereby grants its
consent for the performance of the aforesaid procedure.
