In regards to the vehicle (herein referred to as "vehicle "):

The vehicle was found abandoned on property legally owned by the plaintiff on August 24'
2003. In its condition, the vehicle had apparently been abandoned decades prior to 2003 — possibly by
the previous landowner. The last registration sticker to be affixed to the plate is dated 1985.
Attempts to procure Vehicle Information from PA DOT via form DL -135 are ineffective due to
the age of the information and PA DOT's policy of not retaining records for a period of more than ten
years. A VIN check was performed at the National Insurance Claims Bureau (NICB) and returned no
theft or total loss records.
As Plaintiff, I offer this statement of affidavit that I have diligently and duly reported that this
abandoned vehicle has been documented as abandoned on my property for a period of over 10 (ten)
years. I hereby request that (given the utterly abandoned state of the vehicle) that ownership and title
be awarded to the Plaintiff by completion of the court order on the following page.

were reported as stolen within the past five years and remain unrecovered or declared as
salvage. This information his !being made available, free of charge, as a public service to
consumers.

AND NOW, this 18th day of March, 2014, upon consideration of Curtis A.
Mittong's Petition for Involuntary Transfer of Vehicle Ownership Pursuant to 75
Pa.C.S.A. §102, it appearing to the Court that the vehicle has been abandoned for in
excess of 20 years,
IT IS HEREBY ORDERED AND DIRECTED that ownership of the 1965
International Harvester Scout 800 bearing Vehicle Identification Number: FC153115A is
awarded to Curtis A. Mittong and the right, title and interest of any other person to said
vehicle is hereby extinguished.
IT IS FURTHER ORDERED AND DIRECTED that the Commonwealth of
Pennsylvania, Department of Transportation may accept this Order of Court as
evidence of ownership in lieu of a certificate of title. The Petitioner shall submit any
appropriate forms, taxes and fees and comply with any other procedures of the
Commonwealth of Pennsylvania, Department of Transportation in order to receive the
appropriate certificate of title for the said vehicle.
