KNOW ALL MEN BY THESE PRESENTS:
That WHEREAS, Thomas F. Ptaszek and Robert S. Ptaszek are the beneficiaries of the Estate of
Barbara E. Ptaszek, deceased; and
WHEREAS, Robert S. Ptaszek is the executor of the Estate of Barbara E Ptaszek, and has
administered the estate in accordance with the terms of the Last Will and Testament of Barbara E Ptaszek and
the applicable requirements of Federal and Commonwealth of Pennsylvania law; and
WHEREAS,Thomas F. Ptaszek and Robert S. Ptaszek wish to memorialize the administration of the
Estate and its conclusion in a manner other than by way of formal accounting;
NOW,THEREFORE, in consideration of the distributions of property, cash and securities as
heretofore made and proposed to be made by the Executor,Thomas F. Ptaszek and Robert S. Ptaszek do hereby
waive any formal accounting,judicial or otherwise, of the period of administration of the Estate of Barbara E.
Ptaszek, from her date of death, 29 November 2012 to 22 February 2014, and do for themselves, their heirs,
executors, administrators, successors and assigns, hereby release Robert S. Ptaszek individually, and as
Executor under the Will of Barbara E. Ptaszek, from any and all liability, accountability, or responsibility which
he ever had, now has, or hereafter may or can have for any act of commission or omission (except for acts of
intentional impropriety or gross negligence, if any) in the administration of the Estate of Barbara E. Ptaszek.
