It is hereby ORDERED and DECREED that NEIGHBORHOOD SERVICES is appointed Plenary Guardian of the Person and Estate with powers and duties including, but not limited to the following: (1) to consent to medical treatment; (2) to provide for the continued care and placement of GLADYS SHUGHART, including to consent to and authorize admission to a long-term care or other appropriate care facility; (3) to oversee her care at any long-term care or other facility; (4) to complete and sign any necessary insurance and Medical assistance forms; (5) to ascertain the amount and extent of any assets of GLADYS SHUGHART and to use such funds as necessary to fulfill the preceding duties; and (6)to provide for the continued care and well-being of GLADYS SHUGHART, all in accordance with the provisions of 20 Pa.C.S.A. §5501 et se�c. and as limited therein. It is further ORDERED that the Guardian of the Estate shall not post a Court-appointed bond. , The Guardian of the ESTATE shall have the authority and responsibility to manage and use the incapacitated person's property primarily for her benefit in accordance with 20 Pa. J C.S.A. 5536(a). The Guardian of the ESTATE is permitted to spend INCOME for the care and maintenance of the incapacitated person without the Court's written approval. It is further ORDERED that, as needed to maintain services for GLADYS SHUGHART the Guardian of the Estate shall take all actions necessary to obtain/and or maintain medical insurance for GLADYS SHUGHART including determining Estate assets and make application for insurance and other benefits under the Medical Assistance Program. It is further ORDERED that the Guardian shall be paid an initial fee of$550.00 from the Petitioner. The Guardian shall also be paid $247.2U per month for guardian services rendered; $100.00/month payable from Medical Assistance benefits (should GLADYS SHUGHART be eligible) and the remaining $147.20/month to be paid by the Petitioner. It is further ORDERED that the Guardian of the Person shall file a Report on the social, medical and other relevant conditions at least once within the first twelve (12) months of his appointment and least annually thereafter as required by 20 Pa.C.S.A. §5521(c). It is further ORDERED that the Guardian of the Estate shall file an inventory within three (3) months of this decree of the incapacitated person's real and personal property pursuant to 20 Pa.C.S.A. §5521(b). In addition, the Guardian of the Estate shall file a report with the Court at least once within the first twelve (12) months of his appointment and least annually thereafter as required by 20 Pa.C.S.A. §5521(c). Any Will of GLADYS SHUGHART shall be presented to the Court for its inspection at the time of filing of the report. Such Will shall not be available for public inspection. It is fur�her ORDERED that to the extent the assets of the Estate of GLADYS SHUGHART are not identified at the time of the hearing, this information shall be provided to the Court no later than at the time of filing, and revisions to any Guardian bond requirements shall be made at that time. It is further ORDERED that within sixty (60) days of the death of the incapacitated person or an adjudication of capacity and modification of existing Orders, the Guardian shall file a final Report with the Court. 20 Pa.C.S.A. §5521(c)(2). Neighborhood Services having now been appointed Guardian of the PERSON and ESTATE of GLADYS SHUGHART, determines it appropriate to exercise the authority granted under 20 Pa.
